ACCEPTANCE: With respect to the equipment specified herein, this agreement contains the entire agreement of the parties hereto and supersedes any contract or agreement of prior date between the undersigned Renter and the Company and is subject to final acceptance by the Company. Renter acknowledges receipt of a copy of this agreement and agrees that acceptance by the Company will be shown by its invoice.

RENTAL HIGHLIGHTS

1. I understand that the equipment I am renting is to be returned to The Atlanta Equine Clinic within 5 calendar days of the expiration date of my rental term as selected above.
2. I understand that if equipment is not returned within 5 calendar days of the rental term expiration, automatic extensions of 2 weeks at a rate of $275.00 will be charged to my account (read “Automatic Extension” below for details).
3. PAYMENT: All rentals are payable in advance, beginning on the effective date of delivery, by credit card. Cash and/or checks will not be accepted.
4. EXTENSION OF RENTAL TERM: Renter may opt to extend the rental term for a minimum of 7 days at the respective rates designated above. Renter MUST notify The Atlanta Equine Clinic to extend the rental term. A separate rental extension order will be placed with the corresponding payment applied. Rental extensions may be verbally requested and confirmed.
5. AUTOMATIC EXTENSION: The equipment is due back within 5 calendar days of end of the rental term. An automatic extension of 2 weeks at the rate of $275.00 will be applied to customer’s credit card if the equipment is not returned within the 5 days. The extension will begin the day after rental period expiration. Automatic extensions will be applied at two-week intervals until customer returns the equipment to The Atlanta Equine Clinic.
6. DAMAGE INSURANCE: The risk of loss, injury, or destruction of the above equipment from any cause whatsoever, at all times subsequent to the delivery thereof, is hereby assumed by the Renter, and such injury or destruction shall not operate in any manner to release the Renter from the obligation to make the payments aforesaid and renewal or extension in the terms of this agreement. Renter shall maintain equipment in good working order. The optional insurance fee of $50.00 will eliminate any charges for superficial damage to the system. If the system is not returned in working order, the Renter will purchase the system at a cost of $2500 US.
7. SHIPPING: A flat fee of $95.00 is charged for shipping costs. All rentals are shipped for overnight delivery to the customer. Rental orders received by 11:00am (Eastern Time) at The Atlanta Equine Clinic will be processed for shipment that day. Orders received after 11:00am (Eastern Time) will be sent the following day. All rentals are shipped with a prepaid return shipping label for the customer to use in returning the equipment. If the return label is misplaced or damaged, a duplicate return label may be obtained from The Atlanta Equine Clinic at no charge.
8. TAXES: Renter agrees to pay all taxes associated with the delivery of the equipment and such sums shall be payable to the Company upon delivery of the equipment to the Renter. If claiming a tax exemption, Renter must provide the Company with a valid tax exemption certificate prior to invoicing.
9. PROVISIONS AS TO USE: This agreement is intended by the parties as an agreement to rent and not to sell, conditionally or otherwise. The equipment rented is and shall remain the property of The Atlanta Equine Clinic, and Renter shall have no right, title, or interest therein.
10. DURATION OF AGREEMENT: This agreement shall remain in force from the date of delivery and initial billing until entire system is returned to The Atlanta Equine Clinic. The Renter will deliver the above equipment to The Atlanta Equine Clinic in good condition, except for normal wear and tear. Any pieces of the equipment not returned will be charged to Renter at the then published retail price.
